Ibandronate Sodium is used to cure or prevent osteoporosis. Osteoporosis is a disease which causes bones to turn thin.  Boniva (Ibandronate) resides to a group of medicines called bisphosphonates. It refines the cycle of bone formation and disintegration in the body. Ibandronate works by slowing bone loss and on the other hand increasing bone mass, which prevents bone fractures. Boniva is mostly used to treat or prevent osteoporosis in women after their menopause.

Drug Description:-

Boniva (Ibandronate sodium) is a nitrogen-containing bisphosphonate which constrains osteoclast-mediated bone assimilation. The chemical name for Ibandronate sodium is 3-(N-methyl-N-pentyl) amino-1-hydroxypropane-1,1diphosphonic acid, monosodium salt, and monohydrate. The molecular formula for the same is C9H22NO7P2Na•H2O with molecular weight as 359.24. It is a white or off-white powder type. It is freely soluble in water, whereas practically insoluble in organic solvents.

The formula is available as a white, oblong, 2.5 mg film-coated tablet for daily oral use or as a white, oblong, 150 mg film-coated tablet used once-monthly for oral administration. One 2.5 mg film-coated tablet contains 2.813 mg Ibandronate monosodium monohydrate, equivalent to 2.5 mg free acid. One 150 mg film-coated tablet contains 168.75 mg Ibandronate monosodium monohydrate, equivalent to 150 mg free acid.

Inactive Ingredients:-

Inactive ingredient in the solution consists of: lactose monohydrate, povidone, microcrystalline cellulose, crospovidone, purified stearic acid, colloidal silicon dioxide, and purified water. Tablet film coating contains: hypromellose, titanium dioxide, talc, polyethylene glycol 6000 and purified water.

Indication:-

Boniva (Ibandronate sodium) is indicated for treating & preventing osteoporosis in postmenopausal women. It increases bone mineral density (BMD) and reduces the incidence of vertebral fractures.

Dosage:-

The dose of Boniva (Ibandronate sodium) is either one 150 mg tablet which is consumed once monthly on the same date each month or one 2.5 mg tablet which has to be consumed once daily.

The suggested dose of injection for the treatment of postmenopausal osteoporosis is 3 mg for every 3 months used intravenously over a period of 15 to 30 seconds. Do not use it more frequently than once in every 3 months.

Overdose:-

In case of overdose, drinking a full glass of milk is highly recommended. Do not make yourself vomit forcefully or do not lie down. Overdose symptoms may include nausea, heartburn, stomach pain, diarrhea, muscle cramps, numbness or tingling, tight muscles in your face, seizure (convulsions), irritability, and unusual thoughts or behavior.

Missed Dosage:-

Once Daily - If you forget to take this medicine first thing in the morning, do not take it later in the day time. Wait until the following morning to take the medicine and skip the missed dose. Do not consumed two tablets in one day.

Once a month - Take the missed dose first in the morning on the day after you remember. Then return to your normal monthly schedule on chosen dose day. If your next scheduled dose is less than 7 days away. Then wait for you next coming dose time and skip the missed dose. Do not consume two tablets in one week.

Using guide:-

  • Boniva formula is consumed either once each day or once each month. The solution is given as an injection into one of veins every three months. You can consume the tablets at home.
  • Consume the tablet first in the morning, at least 1 hour (60 minutes) prior you eat or drink anything. If you take a tablet only once a month, make sure that you consume it on the same day each month and always in the morning.
  • Each tablet is to be consumed with a full glass of plain water.
  • Avoid crushing, chewing, or sucking of the formula. Swallow the pill as whole.
  • After consuming tablet, carefully follow these instructions:
  • Avoid lying down or recline for at least 60 minutes after consuming the formula.
  • Do not eat or drink anything except plain water with the pill for at least 60minutes.
  • Avoid consumption of any other tablets counting with vitamins, calcium, or antacids for at least 60 minutes.

Storage of the Drug:-

The drug is to be stored at room temperature away from moisture and heat. At approx. 25°C (77°F); excursions permitted between 15° and 30°C (59° and 86°F)

Side Effects:-

The most common side effects of the drug when consumed in form of injection are bone, muscle and joint pains, flu like illness and headache. The flu like symptom is nothing to worry about as it generally disappears after 24-48 hours and usually occurs only after the first injection. Other common side effects are diarrhea, pain in the extremities, and upset stomach.  Less common side effects occurred after consumption may be pain or severe swallowing, heartburn and stomach ulcers.

Rarely patients have complained major bone, joint and muscle pain that starts from 1 day to one month after starting any bisphosphonate, including Ibandronate Sodium.  Many patients report comfort from these symptoms after stopping the medication.

Precaution:-

Avoid Boniva tablet if you are unable to sit upright or stand for at least one full hour, as it can cause major issues in stomach or esophagus. You will need to stay upright for minimum 60 minutes after consuming this medication.

Do not take any other medicines which include vitamins, calcium, and antacids for at least 60 minutes prior consuming this tablet. Avoid milk and other dairy products for 60 minutes (except in the case of overdose as stated above).
